THRIVINGInPLACE  

Do you want to remain in your home as long as possible?  Are you concerned about being readmitted to the hospital?  Are you concerned about maintaining a solid wellness routine at home? Our THRIVINGInPLACE wellness home care program program was created to address the issues that cause you to be readmitted to the hospital and the issues that may make it challenging to maintain your wellness state at home.

  •   Medication monitoring and reminders
  •   Doctor Appointment monitoring, reminders, and transportation
  •   Weight loss and gain monitoring
  •   Home Safety audit and written suggestions
  •   Daily Exercise and assistance with range of motion
  •   Nutrition Management including shopping, diet adherence, meal preparation, and assistance with feeding (if required)
  •   Hydration administration of appropriate fluids
  •   Mental Engagement, socialization and companionship

And of course, assistance with personal care and household tasks, including bathing, dressing, toileting, and light housekeeping and laundry.

MEMORYCarePlus  

Have you noticed recognizable memory loss by a loved one?  Has a loved one been diagnosed with a form of memory loss like Alzheimer's Disease or dementia?

Are you concerned about a loved one’s safety at home? Are you wanting to extend the time that your loved one can stay at home?  Our MEMORYCarePlus home care program was created to address your concerns.

  •   Medication monitoring and reminders
  •   Home Safety audit and written suggestions
  •   Ambulation assistance and fall risk prevention
  •   Personal care and hygiene assistance and monitoring
  •   Nutrition & Hydration Management including shopping, diet adherence, meal preparation, and assistance with feeding
  •   Mental engagement through conversation, socialization, and companionship
  •   Memory enhancement using pictures, music, and other methods
  •   Clutter clean-up and household organization
  •   Transportation to and from appointments
  •   Driving Assessment with a specially trained professional

And of course, assistance with household tasks, light housekeeping and laundry.

Recommended minimum schedule 2-3 times a week for four hours.

COMFORTPLUS   

Are you receiving palliative or hospice care in your home?  Do you want to remain in your home, but at the same time have concerns about a possible unnecessary hospitalization?  Our COMFORTPlus home care program was created to provide support to clients on their end-of-life journey.

You can expect us to:

  •   Listen as you share your thoughts and concerns
  •   Provide support and companionship to avoid feelings of isolation
  •   Be available for a comforting touch if welcome
  •   Monitor and communicate expressed or observed pain to family and medical professionals (changes, acute, chronic)
  •   Provide repositioning to guard skin integrity if bedridden
  •   Offer Dietary Support through the preparation of meals, nutritional shakes, and assistance with feeding if required
  •   Ensure Hydration through the administration of liquids, crush ice, etc.
  •   Coordinate daily activities and ambulation that can be tolerated
  •   Assist with medication administration* as required
  •   Document observations for family and medical professionals

And of course, assistance with personal care and household tasks, including bathing, dressing, toileting, light housekeeping and laundry.

*It is preferred that a family member or licensed professional set-up and administer narcotics. 

VeteransValourCARE

Veteran’s Aid and Attendance Benefit or Pension

  • Are you or your spouse a veteran?
  • Are you currently in need of in-home caregiver assistance?
  • Are you aware that as a veteran, you may be eligible for Aid and
    Attendance benefit/pension that may cover this care at no cost to you?
  • Would you like StaffLink’s assistance in finding out if you are eligible and assisting you in meeting the “care in place” requirement?

Here are some of the qualifying requirements, some call these the three (3) M’s of qualifying:

  • Military Requirement-The veteran must be considered a “wartime veteran”.
  • Medical Requirement-The veteran must require help with “activities of
    daily living”.
  • Money Requirement-The value of your home and the car ownership are generally not eligibility hindrances. Assets that are not considered “excessive” are also not problematic.

The 2021 annual pension is capped as follows and covers an estimated hours of care per week:

  • Solo Veteran
    $23,232 - 17 hrs per week
  • Veteran with Spouse
    $27,540 - 20 hrs per week
  • Spouse of Living Veteran
    $18,240 - 14 hrs per week
  • Surviving Spouse
    $14,928 - 11 hrs per week

How can our StaffLink’s VeteransValourCARE Home Care Program assist you in applying and becoming eligible for the pension?

  • First, StaffLink will connect you to a reputable local claims consultant. They will have an initial conversation with you to determine the likelihood of your eligibility.
  • If you appear to be eligible, they will assist and instruct you with the application process.
  • Most importantly, if you appear to be eligible for the pension, and you do not have the resources to pay for the “care in place” that is required during the application process, we will connect you with a claims consultant who offers an interest-free loan.
  • If StaffLink is your home care agency of choice, we will start providing care immediately so you can demonstrate that you have “care in place” during the application process.
