What is Exactly is Private Duty Home Care or Private Duty Nursing?

The term “private duty” is sometimes used interchangeably with private pay home care.   Frequently the term is followed by the word “nursing” and called private duty nursing.  Private pay home care means that the client pays for care privately.  Private duty usually means care that is paid for by the client.  Clients usually pay for care privately via check, account debits or credit card.

Some private pay clients opt to hire individual caregivers if they are paying privately.

Private Duty Nursing Care

To the surprise of some, private duty is NOT always paid for privately.  In the state of Missouri, the term private duty nursing (PDN) is also used to describe 8-12 hour shift care provide to Medicaid qualified individuals by a licensed practical nurse (LPN) or a registered nurse (RN).  The majority of those clients are medically fragile individuals are under the age of 21 years.  Medicaid PDN services are usually authorized through the Bureau of Special Health Care Needs.  The states Healthy Children and Youth is the umbrella program.  Persons 21 years and older may qualify for the PDN program under the Medically Fragile Adult Waiver program.

The Missouri state PDN program is having significant challenges.  The St. Louis Post-Dispatch reported in March of 2020 that the PDN program is being sued by several families.  The program is under-funded and the hourly bill rates are not competitive with other industry rates, so the staff pay rates are very low and it is difficult to recruit qualified licensed (RNs and LPNs) staff.
